Dear,
Thank you supply live555, and I use it for a program receive stream media, and save them to a mp4 file.
I know live555 can't convert file format(Am I wrong?). So there are h.264 video and aac audio in RTSP's server. Use order "openRTSP -4 rtsp://... > test.mp4" to receive stream media. When the end, the QuickTime can't play the mp4 file, why?
What should I do? Thank you!

PS: My English is so bad, don't laugh me.

Unfortunately, I don't know. However, it's very important that you specify the correct video frame rate, width and height - using the -f -w and -h options - see http://www.live555.com/openRTSP/#quicktime

You might also try playing your file using VLC: http://www.videolan.org/vlc (VLC can often play files that QuickTime Player cannot.)

Is it really this simple to generate an MP4 file?  It is simply a serialized h264 stream?  I don't need some other library in order to create an MP4 file?

No it's not. A "serialized H.264 stream" is a '.264' or '.h264' file. A '.mp4' file has a completely different format (though it often contains H.264 video).
